The mission of TLC Home Hospice is to provide Hospice Care to Patients with a limited life expectancy and emotional support to their families. The primary goal of this Hospice organization is to improve “quality of life” when the quantity of life is limited. We go above and beyond the care that is needed for our Patients to ease their suffering, and ensure their stage at the end of life is as comfortable and peaceful as possible.
TLC Home Hospice was founded in 1994. We are a State Licensed and Medicare Certified Hospice agency. At TLC Home Hospice, we are committed to providing the best Hospice and Palliative care experience available. Our organization provides care and services according to accepted standards of practice and current law and regulations. We offer Hospice Care to Patients and their families in private homes, Assisted Living Facilities and Skilled Nursing Facilities. The TLC Home Hospice program adheres to the Hospice principles as promulgated by the National Hospice and Palliative Care Organization. Our underlying philosophy is to meet the wishes of terminally ill Patients and their families to live out life to the fullest measure possible with dignity, comfort, alertness and pain free. The Hospice Circle Of Care
The Hospice Circle Of Care depicts the holistic continuum of care rendered by our Hospice Care team as they work together to assess and meet the Patients and families unique needs as the end of life draws near.

Professionally accepted standards are used to guide TLC Home Hospice in the development of clinical and operational policies as well as the procedures and the delivery of care to Patients. It is our goal to ensure that Patients are cared for in a safe environment utilizing proper materials, medications, equipment and treatment techniques. An environmental and safety assessment is completed at the time of admission and Patient education needs are identified.

Eligibility for Hospice Care
Hospice accepts Patients on the basis that the Patient’s medical, nursing and psychosocial needs can be reasonable met by TLC Home Hospice in the Patient’s place of residence. The Patient must have a limited life expectancy (6 months or less if the terminal illness runs its normal course) as certified by the attending physician and the Hospice physician.

Hospice Service Hours
TLC Home Hospice will send out a registered nurse to evaluate and educate the Patient and family shortly after we receive a phone call for help. Part of our service is also to set up daily visits to prevent crisis. We design each Patient's care plan according to the Patient's needs and wishes without being intrusive. TLC Home Hospice has a registered nurse on call 24 hours a day, 7 days a week. We make sure we are available to our Patients and families when they need us the most.

Hospice services are covered by Medicare and Medicaid.
