The Moorpark Health Care Center (formerly the Tender Loving Care Facility) is a 10-bed congregate living health facility featuring private and semi-private rooms. The facility is a 5,000 square foot house located in a quiet residential neighborhood not far from the TLC Home Hospice headquarters in Moorpark. Moorpark Health Care Center accepts long-term care patients requiring skilled nursing care and observation on a 24-hour basis, and high acuity level patients. Loving Heart Hospice Foundation was founded in 1996 to champion the benefits of hospice care and provide financial assistance to those low-income patients and families facing a shortfall in hospice care. The Foundation also funds educational programs to reach the community and doctors about the benefits of hospice for the terminally ill.
