Services: Our beautiful
4700 sq. ft. custom home facilitates 10 beds, offering
24 hour skilled nursing care in a home setting. State of
the art surveillance equipment is employed 24 hours a day.
We give patients and families an alternative to Skilled
Nursing Facilities. You and your loved one can enjoy the
comforts of home at our facility. Other services are dietitian,
physical therapy, recreation activities, and outdoor accommodations. |

Eligibility: To be eligible for admission to TLC Facility patients have to meet specific diagnosis criteria: 1) A life threatening illness with a life expectancy of 5 years or less; or 2) A terminal illness with a life expectancy of 6 months or less. |
