Same Day Service/ Daily Visits

TLC Home Hospice will send out a registered nurse to evaluate and educate the patient and family and shortly after, we receive a phone call for help. Part of our service is also to set up daily visits to prevent crisis. We are not intrusive and will design the care according to the patients needs and wishes.

24 hour on call service

TLC Home Hospice has a registered nurse on call 24 hours a day, 7 days of the week. We make sure we are available to our patients and families when they need us most.

Pain Control Management

Our Hospice Nurses are experienced and trained to be the experts in pain management. We strive for all patients to have quality of life and comfort care. Our Medical Directors are highly experienced in pain management and will do house calls whenever needed. We do not hasten or prolong death. Many times we find that when a patient is comfortable and at peace they decide when is the best time to go.

Hospice 4 Levels of Care

ROUTINE CARE

Care that can be given at home or in a facility; non-acute care. Unlimited number of days as long as Hospice eligible.

CONTINUOUS CARE

Highest acuity level at home for crisis management as long as medically necessary and until crisis in controlled. Usually for a short period of time on a 24-hour basis. Unlimited number of days as long as medically necessary.

GENERAL INPATIENT

Highest acuity level in a 24-hour skilled nursing facility (SNF) for conditions requiring nursing intervention on a 24-hour basis for a short period of time. Patients are usually transferred directly from a hospital to SNF. Unlimited days as long as medically necessary

INPATIENT RESPITE

Patient is admitted to a Skilled Nursing Facility to allow the caregiver/family respite. Benefit is for no more than 5 days as approved by the hospice.

About our Medicare Program

TLC Home Hospice is Medicare-certified which allows us to service those patients who are on Medicare. When a Medicare-eligible patient receives services from a Medicare-approved hospice, the Medicare hospice benefit covers 100%.

TLC Home Hospice is JCAHO Accredited. Joint Commission on Accreditation of Health care organizations standards deal with organizational quality of care issues and the safety of the environment in which care is provided.

TLC Home Hospice undergoes periodic surveys by JCAHO and is consistently found to meet the requirements for accreditation and excellence.
