Diversity

TLC Home Hospice has a diverse group of professionals. We call it our hospice Interdisciplinary Team. It consists of physicians, nurses, personal care aids, chaplains, social workers, counselors, professional consultants, volunteers, administrative personnel and Certified Senior Advisors. The team meets weekly and monthly to coordinate the care plan for each patient and family.

Experience

Our experience is diverse as well. TLC professionals have an accumulated 70 years or more experience in health care. This includes marketing, medical administration, medical practice, nursing, social services, and medical financial services. This experience and our commitment to our patients and families accounts for our excellent reputation.

Education

All direct patient care staff have current licenses and certifications with the state of California. Our staff of professionals are highly educated and keep up their licenses and certifications with Continuing Medical Education and Continuing Education Units. Our administrative staff is also educated constantly to provide great service.

Performance

Our philosophy of how services are administered has made TLC Home Hospice the preferred hospice for many physicians, IPA's, medical groups, patients and their families. Our management team has been in place and working smoothly since 1994. We have a Medicare Certified provider Hospice agency in Moorpark serving Ventura and Los Angeles Counties. We experienced a consistent period of growth over the last few years enabling us to open a Hospice facility in the first quarter of 1998. We continually strive for excellence and became Joint Commission accredited in 1999. In 2000, we added Certified Senior Advisors on a full time basis.
